New York Court Forms

Order of protection forms and other family court forms are available online from the State of New York Unified Court System, including the petition to terminate or sever your lease if you are a victim. You can also find the application for an extreme risk protection order if you believe an individual is a danger to himself/herself or others and should not be able to have access to firearms. For certain types of cases, such as a modification or enforcement of a custody/visitation order or support order, you can fill out the forms online. You can also get the forms at the courthouse. You may be able to get help filling out forms by calling a local domestic violence agency or a legal services organization. To find an advocate at a local program or a legal services organization, please visit the NY Places that Help page.
